Puppy Reservation/Agreement


This form is used to secure a pick from one of the litters here at Country Meadow Doodles. After submitting this form, proceed to "Step Two" in our Reservation Process, which is submitting your deposit via Venmo. We order our picks by Venmo timestamp and not Reservation Form/Deposit Agreement timestamp.

Agreement Acknowledgment:

This agreement (“Agreement”), dated as of this, the day this form is submitted (the “Effective Date”), is between Country Meadow Doodles (“Breeder”), and the above mentioned person (“Buyer”). Buyer agrees to place a $250.00 deposit (“Deposit”) on a puppy from Breeder.

Non-Refundable Deposit:

Buyer acknowledges that the Deposit is non-refundable. Upon receipt of the Deposit, Breeder agrees to hold the puppy for Buyer until it has been weaned and is ready for pick up (8-10 weeks). The Deposit shall be made in US Dollars and paid to Breeder via payments through Good Dog or Venmo.

Buyer Acknowledgments:

Buyer further acknowledges and agrees that (a) Breeder cannot control litter sizes and that the wait time for puppies can be longer than expected; (b) the Deposit gives Buyer a “pick” place, but does not guarantee a puppy or that Buyer’s preferences will be available when it is Buyer’s turn to pick; (c) pick positions are given on a first-come first served basis and Buyer’s position in line is not negotiable; (d) Breeder will not reserve a pick position until the Deposit clears into the Breeder's bank; and (e) Breeder cannot and does not guarantee any specific qualities of the puppy when it becomes an adult.

Buyer Right to Transfer:

Notwithstanding the foregoing, if a puppy that meets Buyer’s identified preferences is not available when it becomes Buyer’s turn to pick a puppy from the selected litter, Buyer may elect to have the Deposit transferred to the next litter, or a different litter from Breeder.

Buyer Deposit Accounting:

Breeder and Buyer agree that (a) the Deposit shall be applied to the final purchase price of the puppy and (b) the remaining balance will become due before the puppy ownership is transferred from Breeder to Buyer.

Final Payment Agreement:

I agree and understand that the final payment will be due in full before picking up the puppy. I also agree and understand that I will pay Breeder $100 per week starting on the 8th day after the official Puppy Go Home date to keep my puppy until I am able to pick her up.

Health Guarantee:

(1) Breeder provides a 1-year warranty from the puppy/dog’s date of birth against any debilitating congenital conditions. (2) For purposes of this section, “debilitating congenital conditions” include hip dysplasia, kidney, heart, pancreas, spine and liver problems of a severe, life threatening or altering nature and (3) expressly do not include conditions that may be considered to be common in the Doodle breeds. These include, but are not limited to non-inherited forms of cherry eye, entropion, extropion, umbilical hernia, inguinal hernia, bacterial infections, parasitic infections, fungal infection, stenotic nares, soft palate issues, hypoplastic tracheas, allergies, undescended testicles, ocosmetic issues, other autoimmune disorders. (4) Environmental factors will not be covered by this guarantee. Including allergies, demodectic mange, or issues caused by heat stroke, or the buyer's neglect or ignorance or improper socialization (5) Breeder does not assume responsibility for any reaction to any vaccination. (6) This guarantee does not apply to any physical problems which could be the result of an injury. (7) This guarantee does not apply to the presence of deafness or any kind of hearing problems. (8) This guarantee does not apply to the presence of any vision related problems. (9) In the event the puppy/dog exhibits symptoms of a suspected debilitating congenital condition, Buyer agrees to notify the Breeder of the problem as well as furnish, at the Buyer's expense, veterinary reports by two un-associated licensed veterinarians stating the affliction within two (2) business days of the problem's discovery. (10) The puppy/dog must exhibit the congenital defect within the effective period of this contract. (11) See sections 16-22 (12) If the puppy/dog dies within this contract's effective period: the Buyer agrees to assume financial responsibility for an autopsy on the puppy/dog, to be performed by a licensed veterinarian to determine the cause of death. (13) Buyer agrees to forward the written autopsy report to the Breeder. (14)If the cause of death is found to be any affliction listed as a debilitating congenital condition pursuant to this guarantee, the Breeder agrees to replace the puppy/dog with a similar puppy/dog of the same litter if available, otherwise the next available litter. (15) Buyer agrees that there is no monetary refund of any kind, full or partial guaranteed or implied in regards to section 12. (16) If it is determined by a licensed veterinarian that the puppy/dog “unfit for purchase” or has a debilitating congenital condition that is identified within the warranty periods described above, (17) Buyer may choose to keep the puppy/dog, thereby making void this agreement. (18) Should Buyer choose to keep the puppy/dog and the puppy/dog should require veterinary care/surgery because of the congenital condition identified pursuant this guarantee before the age of one year, Breeder agrees to refund UP TO AND NO MORE THAN the purchase price of the puppy/dog, one time, one payment to help cover Buyer's veterinarian/hospital fees. (19) A puppy/dog eligible for replacement, pursuant to sections two, three of this guarantee, must be returned to Breeder or euthanized with prior permission from the Breeder to qualify for replacement. (20) The returned puppy/dog must appear in good condition to the Breeder, other than the specified congenital problem. (21) A puppy/dog will not be eligible for replacement if it has been or appears to have been neglected, abused, bred or euthanized without prior permission from the Breeder. (22) Buyer agrees to any and all financial responsibility for all expenses whether transportation, medical or miscellaneous incurred while the puppy/dog to be returned is in their possession. (23) No other warranties or guarantees, expressed or implied, are made by Breeder, and the puppy/dog is sold and delivered in an “as is” condition, except as expressly and specifically set forth herein.

Privacy Policy/Legal Notice:

Breeder does not and will not knowingly share any customer data with any party at any time for any reason. This Agreement is of a private nature and jurisdiction. Notwithstanding fraud, or intentional malicious criminal negligence or actions, Buyer waives all rights to litigation and/or arbitration in regards to any dispute that should arise in regards to this Agreement. Any dispute will be settled privately in a private venue. This Agreement is to be considered to be a non-statutory, non-commercial, non-domestic, private agreement between private individuals and is not to be construed as any sort of public action or public event. All communications, interactions, verbal, written or otherwise are private.

By submitting this form, I agree that the above information is accurate and true to the best of my knowledge and understanding. I also agree to the terms listed herein.
